Registration
• A prior appointment is required for any type of consultation at CDC. For taking appointments you may contact in the phone number- 0471-2553540, 0471-2447061 from Monday to Saturday (except second Saturdays and specified public holidays) between 1pm-2pm.
• A referral letter from a doctor (modern medicine) is required at the time of registration
• Please stick to the given time when coming to register the child
• The child’s name, age, parent’s name, phone number, exact address, income and other details are required at the time of registration.
• The treatment details of the child from birth must be brought with the parent
• After consultation, the parents are supposed to fix the follow up date and time
• The clinics at CDC does not function on Second Saturdays, Sundays and on pre-determined public holidays
• After the registrations the patients are called for check-ups and treatment in the order of token numbers. The token numbers will be called upon from 9am. The different check-ups and treatments may be delayed till 3.30. You have to patiently wait till all procedures are over.
• Please take in hand the necessary food and clothes for the child. For small children, please take diapers along with you while you come to CDC.
• If you were unable to come on the date specified for appointment, you can take another appointment by calling in the number -0471-2553540 between 2pm and 3.30 pm on working days.
• If you are a BPL ration card holder please bring that along with you while you make a visit to CDC
